Home from hospital

The home from hospital service is available to vulnerable adults over 18 who live on their own in West Sussex, and in need of some social and practical support after they are discharged from a stay in hospital. This free service, funded by West Sussex County Council, is supported by volunteers who will visit clients regularly for 6 weeks.

Volunteers provide companionship to someone who has little social support, popping in for tea and chat or escorting someone to a local activity or group if they are able. They can help build confidence with walking and getting about around the shops. Alternatively, volunteers can provide practical support to those who are recuperating from their hospital stay by assisting with light shopping, preparing a light snack, picking up prescriptions and accompanying clients to follow-up GP and hospital appointments.

The service aims to support people to become independent again in their own homes, and the team will signpost and refer clients on to other agencies to ensure that longer term support can be put in place if needed.
